Services provided on an intermittent basis under a physician’s plan of care. Include but not limited to the following:
Skilled Nursing
- Evaluate and assess status of patient
- Communicate with physician
- Provide intermittent skilled nursing
- Skilled assessments
- Medication Education, Instruction ,and Management
- Nutrition Instruction
- Injections
- Catheter insertion/care
- Wound Care
- Ostomy Care
- Patient education regarding disease process
- Education on available community services
- Home Infusion ( Chemotherapy, I.V. Antibiotics, Pain Control, Enternal Therapy, Etc.)
- Negative Pressure Wound Dressing
Home Health Aide
- Provide assistance with personal care such as:
- Bathing
- Dressing
- Skin care
- Oral hygiene
- Foley catheter care
- Assist with food preparation, linen change, and limited housekeeping directly related to patient care
Speech Therapy
- Evaluate and treat speech and communication problems
- Instruct in exercises to improve verbal and non-verbal communication
- Instruct on laryngectomy communication
- Evaluate and advise regarding swallowing problems
- Speech articulation and treatment
- Voice and language disorder treatment
Physical Therapy
- Evaluate and treat diagnosed mobility deficits and pain conditions
- Communicates with physician
- Provides skilled therapy for:
- Therapeutic Exercises
- Gait Training
- Pain Control Modalities
- TENS
- Ultra Sound
- Electrical Stimulation
- Patient education as relates to mobility
- Educate patient and family about maintenance program
- Advise on home safety issues
- Instruct in use of various assistive devices for mobility: Walkers, Canes, Sliding boards, W/C’s, Lifts
- Rehabilatation
- Prosthetic Training
Occupational Therapy
- Evaluate and instruct in home adaptation for maximum self-care and safety
- Evaluate and instruct in self-care activities for daily living – dressing, bathing, home making, and personal care
- Muscle re-education
- Adaptive equipment training
